Form Description:

The purpose of these instructions and form is to obtain the information
necessary to enable HUD to make a determination, prior to
issuance of a SAMA or Feasibility letter, that the sponsor of a
mortgagor or the mortgagor itself, if the mortgagor has been created,
is truly nonprofit in accordance with the definition above and demonstrates
probable success in project development and continuing
operation.
